Pharmacologic Classification
Baricitinib is classified as a Janus kinase inhibitor. Its principal pharmacologic activity involves inhibition of JAK1 and JAK2.
Janus kinases are intracellular enzymes involved in transmitting signals from cytokine receptors. Cytokine signaling contributes to immune regulation, inflammation and hematologic processes.
By inhibiting JAK1 and JAK2, baricitinib modifies signaling pathways involved in inflammatory and immune responses.
Clinical Role
Baricitinib has a clinical role in several immune-mediated conditions. The exact indications differ between jurisdictions.
In the United States, current labeling includes:
- Moderately to severely active rheumatoid arthritis in specified adults
- Severe alopecia areata in adults
- Hospitalized adults with COVID-19 requiring specified respiratory support
The U.S. label specifically states that baricitinib is not recommended in combination with other JAK inhibitors, biologic DMARDs or certain potent immunosuppressants for its rheumatoid arthritis indication.
European authorization includes rheumatoid arthritis, atopic dermatitis, severe alopecia areata and specified juvenile idiopathic arthritis.
Rheumatoid Arthritis
In rheumatoid arthritis, abnormal immune activity contributes to chronic synovial inflammation. Persistent inflammation can cause pain, stiffness, swelling and structural joint damage.
Baricitinib reduces signaling through cytokine pathways involved in inflammation. Clinical studies evaluated its ability to improve disease activity and symptoms in patients with inadequate responses to previous treatments.
EMA information reports that studies involving approximately 2,500 adults with rheumatoid arthritis demonstrated improvement in symptoms and disease activity compared with placebo or comparator treatment in appropriate populations.
Atopic Dermatitis
Atopic dermatitis involves inflammatory signaling, impaired skin-barrier function and itching.
Baricitinib’s JAK inhibition can reduce inflammatory signaling and may improve skin symptoms and itch in appropriately selected patients.
European authorization includes adults and children from 2 years of age who require systemic treatment.
Alopecia Areata
Severe alopecia areata involves immune-mediated attack affecting hair follicles. Baricitinib can modify inflammatory signaling associated with the disease.
Current U.S. labeling provides evidence-based treatment information for adults with severe alopecia areata.
European authorization extends to adolescents from 12 years of age with severe disease.
Juvenile Idiopathic Arthritis
European product information includes specified forms of active juvenile idiopathic arthritis in children aged 2 years and older who have had inadequate response or intolerance to prior conventional synthetic or biologic DMARDs.
This is an example of why website content should clearly identify the jurisdiction behind an indication.
COVID-19
U.S. labeling identifies baricitinib for hospitalized adults with COVID-19 who require supplemental oxygen, non-invasive or invasive mechanical ventilation, or extracorporeal membrane oxygenation.
This is a specific clinical setting and should not be interpreted as a recommendation for routine outpatient treatment.
Laboratory Monitoring
Because baricitinib influences immune and hematologic pathways, clinicians may monitor blood counts and other laboratory parameters.
Monitoring can be especially important when patients have underlying kidney disease, liver abnormalities, blood-count abnormalities or other factors affecting treatment.
Infection Assessment
Patients should be assessed for infection risk before and during therapy. Serious infections, including tuberculosis and other bacterial, fungal and viral infections, are recognized safety concerns.
Patients should promptly report concerning symptoms such as persistent fever, unusual cough, breathing difficulty or other signs of infection.
Cardiovascular and Thrombotic Risk
JAK inhibitors have important safety considerations concerning thrombosis and major cardiovascular events. EMA safety information specifically identifies blood clots and major cardiovascular events among risks requiring attention.
Risk assessment is particularly important in patients with established cardiovascular risk factors.
Cancer Risk
Immune-modulating medicines can influence cancer-related risk considerations. Regulators emphasize malignancy as an important safety issue for JAK inhibitors in appropriate patient populations.
The benefit-risk balance should therefore be individualized.
Pregnancy and Breastfeeding
Pregnancy considerations are important with baricitinib. Current European product information states that Olumiant must not be used during pregnancy and recommends contraception during treatment and for at least one week after stopping.
Current U.S. information advises against breastfeeding during treatment and for four days after the last dose.
Clinical Interpretation
Clinical trial results describe groups of patients and cannot predict exactly how an individual will respond.
Baricitinib treatment should therefore be selected and monitored by qualified professionals who can integrate the indication, previous therapies, laboratory findings, comorbidities and safety risks.
